Wrong spellings, a wrong date of birth, a relative’s name recorded incorrectly, an unrecognisable photograph: errors on the electoral roll are common, largely because entries are transliterated and hand-entered.
They are fixed with Form 8, under “correction of entries”. The same form handles address changes, EPIC replacement and disability marking, so the section you tick matters.
What can be corrected
- Your name, including spelling
- Your relative’s name and the relationship recorded
- Date of birth or age
- Gender
- Address details
- Your photograph
You may correct a maximum of four entries in a single application. If more than four are wrong, file a second application for the rest.
Every correction needs proof
Each correction must be supported by a self-attested documentary proof that shows the correct value. The officer is not taking your word for it; they are replacing one recorded fact with another, and need a document that evidences it.
In practice that means a document carrying the correct spelling for a name change, a birth certificate or school certificate for a date of birth, and so on. If you are correcting several things, the proof needs to cover all of them.
How to file it
- Sign in at voters.eci.gov.in.
- Open Form 8.
- Select correction of entries, and only that. Ticking more than one of Form 8’s four purposes gets the application rejected.
- Choose which fields are wrong and enter the correct values.
- Upload proof for each.
- Submit and keep the reference number.
Correcting versus replacing the card
These are different things and people mix them up.
- Correction of entries: the roll itself is wrong. Use this when the recorded information needs to change.
- Replacement of EPIC without correction: the roll is right but the physical card is lost, mutilated, or destroyed by flood, fire or another disaster. The details stay as they are; you are only getting a new card.
For a replacement, a mutilated or old card must be returned. For a lost one, attach a copy of the FIR or police report.

Marking as a person with disability
Form 8’s fourth purpose is PwD marking. Flagging your entry lets the Election Commission provide assistance at the booth: ramps, wheelchairs, priority entry and, in some elections, postal or home voting facilities. It is worth doing if it applies to you.
Before you file anything
Check what is actually recorded. A name that looks wrong to you may be a transliteration the officer considers correct, and a mismatch you noticed on a third-party website may not exist on the real roll at all.
